EC9: Change the identity of the service provider
If you want to apply to change the identity of the service provider operating a licensed early childhood service, you will need to fill out the EC9 application form.
Use the EC9 if you want to:
- Sell or transfer ownership of a service you operate to another legal entity.
- Change the legal name of the service provider.
- Change the service provider ownership type, eg change from a sole trader to a company.
An EC9 Statutory Declaration form needs to be completed by the service provider proposing to operate the service.

Application forms
Email or post your completed form (along with other required documents) to your local Ministry office. You may like to keep a copy for your personal records.
