Following the death of Malachi Subecz, the Chief Executives of 6 agencies, including the Ministry of Education commissioned Dame Karen Poutasi to review ways to improve how our agencies identify and respond to suspected child abuse. Her report was published on 1 December 2022 and is available below.

When the Ministry became aware of Malachi’s death, we undertook a review of the childcare centre he had attended, ultimately leading to the cancellation of the centre’s licence. We also initiated a review of our Ministry processes, and our report on this was provided to Dame Karen to help inform her review.
